25I-NBOMe ist ein hochpotenter 5-HT2A-Agonist mit klinisch relevanter 5-HT2B-Agonismus, die starke periphere Vasokonstriktion verursacht. Stimulanzien (Amphetamin, Kokain) erhöhen Katecholamine und verursachen zusätzliche Vasokonstriktion. Die Kombination erzeugt synergistische Vasokonstriktion mit Risiko für koronare Ischämie, Rhabdomyolyse durch periphere Minderperfusion, zerebrovaskuläre Ereignisse und Hyperthermie. 25I-NBOMe ist mit zahlreichen Todesfällen assoziiert, häufig durch kardiovaskuläre Komplikationen. Die steile Dosis-Wirkungs-Kurve macht die Substanz auch ohne Stimulanzien-Kombination hochriskant.
