JWH-018 ist ein synthetischer Cannabinoid-Rezeptor-Agonist mit CB1-Vollagonist-Eigenschaften — im Gegensatz zu THC (Partialagonist) hat JWH-018 kein protektives Ceiling für die ZNS-Depression. Die maximale ZNS-depressive Wirkung übersteigt die von Cannabis deutlich. In Kombination mit Depressiva (Benzodiazepine, Opioide, Alkohol, GHB) addiert sich die ZNS-Depression ohne das bei Cannabis beobachtete Sicherheitsprofil. Synthetische Cannabinoide sind mit Krampfanfällen, Bewusstlosigkeit und akuter Nierenschädigung assoziiert — Risiken, die bei natürlichem Cannabis nicht auftreten.
