Lebensbedrohlich: Extreme ZNS-Depression durch kurzwirksames Benzodiazepin und Alkohol

Alprazolam ist ein hochpotentes, kurzwirksames Benzodiazepin, das als positiver allosterischer Modulator am GABA-A-Rezeptor wirkt. Ethanol verstärkt ebenfalls die GABAerge Neurotransmission und wirkt zusätzlich als NMDA-Antagonist. Die Kombination führt zu einer überadditiven Verstärkung der ZNS-Depression mit erhöhtem Risiko für Bewusstlosigkeit, Atemdepression und Tod. Die schnelle Anflutung von Alprazolam macht diese Kombination besonders unberechenbar.

GABA-A-Allosterische Modulation (Alprazolam) + GABAerge Wirkung + NMDA-Antagonismus (Ethanol) = überadditive ZNS-Depression

Harm Reduction

• Die Kombination von Alprazolam und Alkohol ist in der Fachliteratur als kontraindiziert beschrieben. Bereits geringe Alkoholmengen können die sedierende Wirkung erheblich verstärken.

• Symptome einer schweren ZNS-Depression — verlangsamte oder flache Atmung, Bewusstseinstrübung, Zyanose — erfordern notärztliche Sofortversorgung.

• Bei Bewusstlosigkeit: Stabile Seitenlage herstellen und sofort den Notruf (112) kontaktieren.
