Warnings & Contraindications

Pre-existing structural cardiac abnormalities are a contraindication — sudden cardiac death has been reported; cardiac evaluation before prescribing in at-risk patients
MPH + MAOI: hypertensive crisis risk — MAOI washout period of at least 14 days required
Abuse potential increases dramatically with non-oral routes — insufflation or injection produces amphetamine-like effects with high addiction liability
Growth suppression in children with long-term use — treatment breaks and height/weight monitoring are recommended
Psychosis can be induced or unmasked at high doses — screen for personal/family history of psychotic disorders before prescribing
Take the lowest effective dose; work with a prescriber to find the minimum effective dose rather than escalating
Avoid taking late in the day — insomnia is dose- and timing-dependent
Do not crush extended-release formulations (Concerta) — releases full 12-hour dose immediately
