Bei Freizeitdosen wirkt GHB als vollständiger Agonist am GABA-B-Rezeptor (Gi-Protein-gekoppelt: K+-Kanal-Aktivierung, Ca²+-Kanal-Inhibition). Dies erklärt die sedierenden, euphorischen und amnesestischen Effekte. Bei sehr hohen Dosen zeigt sich positive allosterische Modulation am GABA-A-Rezeptor. Das extrem enge therapeutische Fenster (1g Unterschied zwischen Sedation und Bewusstlosigkeit) macht GHB besonders gefährlich.
