MDMA is a non-selective monoamine releaser and reuptake inhibitor. Its primary mechanism involves TAAR1 activation, which triggers PKA/PKC-mediated phosphorylation of monoamine transporters (SERT >> NET > DAT) and reverses transporter flow, causing substantial serotonin, norepinephrine and dopamine release into the synaptic cleft. MDMA is a substrate for SERT, NET and DAT (carrier mechanism), not a pure reuptake inhibitor. It also inhibits VMAT2-mediated vesicular storage and directly depletes monoamine stores. 5-HT2A agonism contributes to entactogenic and empathogenic effects.
